Dear Patients: It is with mixed emotions that I announce my resignation from Women’s Health Specialists of Dallas, effective December 9, 2021.
Dr. Jonathan R. Brough, Dr. Angela M. Angel, Dr. Kavitha Blewett, and Dr. Marianne Ebrahim will take over my patients and continue to provide your care unless you request otherwise. I am confident in their abilities to care for you and your needs. If you plan to continue with this office, you may call (214) 363-4421 to schedule your next visit with Dr. Brough, Dr. Angel, Dr. Blewett, or Dr. Ebrahim, or our Nurse Practitioner Myesha Johnson. Your records will continue to be on file at Women’s Health Specialists of Dallas.
You may decide to seek medical care from another provider. If you choose to do so, I recommend looking for a new provider as soon as possible. The office staff at Women’s Health Specialists of Dallas can assist in your search by providing the names of providers accepting new patients. Your medical records are confidential, and a copy can be transferred to another provider or released to you or to another person you designate only with your permission. If you choose to see a different provider, please sign the authorization form available on our website and return it to the office as quickly as possible so we may transfer your records.
